Application to the Leadership Mountain View Program

Candidacy is open to all individuals eighteen years and older regardless of race, creed, sex, disability, philosophy or employment status. Applicants must be residents of Mountain View, representatives of Mountain View based businesses or organizations, or otherwise active in the Mountain View Community. Applicants in final consideration will be personally interviewed.

Application's are currently being accepted for the class of 2009. Completed application and a letter of recommendation must be received by July 30, 2008. Complete the application below.

RECOMMENDATION
Please ask a person who is not related to you, but who is familiar with your ability and accomplishments, to write a letter of recommendation. The letter must be received by Leadership Mountain View by July 30, 2008. The letter should be emailed to Michelle K. Proulx, Executive Director, at mproulx@chambermv.org
TUITION
Tuition is $1,750. All candidates selected for Leadership Mountain View must pay a $200 deposit within one week of acceptance to the program to confirm a place in the class. The balance of the tuition can be paid by one of the following options: Preferred Payment Option: Payment in full within one week of acceptance. Full Tuition Option: Remainder paid by September 1. Pay in Automatic Installments with credit card on file, starting September 1. Mastercard and Visa are accepted. Checks should be made payable to: Mountain View Chamber of Commerce Education Foundation.

FINANCIAL AID

Limited financial aid may be granted on a case by case basis, depending on available funds. Financial aid to participants is based on the following two primary criteria: Applicant's financial need. Applicant's contribution to ensuring a broad spectrum of community representation and participation in the program. For those chosen to receive financial aid, the $200 place holding deposit is still required to be paid within one week of acceptance to the program.
APPLICATION FEE
There is a $30 non-refundable application fee due at the time of application. If applicant is accepted into LMV, a portion of your registration fee is applied to your tuition. The cost of the class is $1750. REFUND POLICY
Withdrawal from the program following confirmation to participate will result in forfeiture of tuition unless an alternate representative is confirmed and accepted as a replacement in the class, prior to its first session.
ATTENDANCE POLICY
In order for Leadership Mountain View to accomplish its goals, the full participation of each individual on the designated Friday of each month is mandatory. The following will result in dismissal from the program: Absences in excess of the equivalent of two full program days. Each tardy is regarded as 1/2 equivalent program day absence.
